SCHEDULE AN APPOINTMENT

To schedule an appointment with Cornerstone Prosthetic and Orthotics find your closest Cornerstone location and give us a call today. Here's what you can expect. Appointment #1: At your first appointment, your practitioner will evaluate, measure, and may take a cast mold of you. The casting process consists of a procedure using plaster or fiberglass wrap (which is removed after it hardens). Your appointment is complete at that point. Prior to your next visit, your cast will be used for the fabrication of a custom device. Casting is occasionally postponed to another appointment for various reasons. You will not receive a custom device on your first visit, however prefabricated devices are often provided on the first visit.

How to Schedule an Appointment

1 / Prescription

Obtain a signed prescription (Rx) from your physician (MD or DO)

2 / Medicare Insurance

If you have Medicare and were prescribed one of the devices listed below, please Download and print the appropriate PDF Dear Physician Letter from the links below. Be sure you take this letter with you to your next physician appointment.

Your physician will then need to provide you with a copy of their chart notes (or they may be faxed to Cornerstone).


Medicare requires that the physician's chart notes follow specific guidelines for the prescribed device.

Once your appointment is scheduled, please bring the following and show up 15 minutes early for registration:

  • a. photo ID

  • b. insurance card

  • c. prescription (Rx)
